Your First Visit

On your first visit to Trinity Endodontics, please bring the following items to help us see you promptly:

  • Dental Insurance Card (if any)
  • Driver's License or Photo ID
  • Completed Patient Forms (See the Patient Forms page)

Patient Forms

For new patients to our office, please visit our Patient Forms page to download our new patient bundle that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

We want to provide superior professional care and personalized service at the most reasonable cost to our patients. Payment for care is required at the time treatment is provided. For your convenience, we offer a variety of payment options, cash, checks, and most major credit cards (VISA, MasterCard, Discover Card, American Express). For information on insurance coverage, please visit our Financial Policies page.

We also accept CareCredit, an external financing option that allows you to pay over time for the treatment you need today. Every CareCredit transaction is eligible for a No Interest Plan or Low Interest Payment Plan, and you can use the card at all healthcare practices that offer CareCredit. The CareCredit card can be used for any dental/medical bills of $300 or more. For more information and to apply online, visit the CareCredit website.
